Below is a form that is for any walk-on / tryout athlete needs to fill out completely and return to the Fort Lewis athletic training room PRIOR to participating in trying out for any varsity athletic team.

Home Coverage:
Home events will be covered by an BOC Certified Athletic Trainer and assisted by athletic training students. Ambulance coverage will be available at football games, as well as an orthopedic physician. A physician will be present or on call at soccer games.
Athletic Training Room:
The athletic training room is open Monday through Friday 8:00 a.m. to 11:00 p.m. It is also open 1 ½ to 2 hours prior to events and after events as needed. Contact the athletic training room at 970-247-7576 if special arrangements are necessary.
Emergencies:
Physicians and an ambulance are on site for football games. For all other events, these services are on call and available.
