Information regarding initial eligibility and forms that must be completed before enrolling ing a Division I or Division II college.

Pre-Participation Forms
These are forms that must be filled out before getting on the field/court. The Fort Lewis Skyhawk Athletic Training staff must have completed forms before allowing any student-athlete to participate. Please read carefully and make sure to fill out the correct forms.

Sports Information Form
This forms allows Fort Lewis College and Skyhawk athletics to use your name, image and additional information provided in press releases, media guides and other publications (ex. website). Please complete the form and return before preseason.

Prospective Student-Athlete Questionaire
This online form allows coaches to get quick information on interested athletes and allows interested athletes a quick option as well.
