To Fort Lewis College Student-Athletes:
Below are two forms that you will need to fill out completely and return to the Fort Lewis athletic training room. ALL student-athletes must fill out the substance abuse policy and one additional form. Please read carefully and return both forms.

Sports Information
The Skyhawk Sports Information office handles all media requests, all components of the Skyhawks website, statistics for every sport, Skyhawk press releases and numerous other tasks. For more information please contact current Sports Information Director (SID), Andrew Beardsley at (970) 247-7441. This forms gives the Sports Information office vital information that gives the Skyhawk Atheltic Department the ability to feature student-athletes in press releases, media guides, on the website and in numerous other publications.

Compliance Information
Every student-athlete must stay eligible during and out of season in order to continue competing. The Skyhawk Compliance office, recently taken over by Hollie Stanley, tracks student-athlete eligibility and maintains NCAA Compliance across the department.
Skyhawks on the Radio
Fort Lewis games will be broadcast on KIUP (930 AM), one of three stations in the Four Corners Broadcasting family. Other FCB stations include KIQX (101.3 FM) and KRSJ (100.5 FM).
The relationship between KIUP and the Skyhawks is among the longest-running athletic broadcasting committments in Colorado, spanning more than 60 years. KIUP first began covering Fort Lewis athletics in the middle 1940's.
